1. Select Complimentary Phone Consultation or In Office-Consultation depending on your availability and preferences.
2. Your appointment will be confirmed by Dr. Stenberg, and an intake form will be sent to your email address. This MUST be completed online prior to your appointment* time, and takes approximately 10 minutes to complete online.

4. Please direct any questions or concerns via TEXT MESSAGE to (719) 301-9650 for a prompt response.

Cancellation Policy: A $50 rescheduling fee is required to rebook an in-office cancellation for same day cancellations. This fee will be applied to future services at your next appointment, or credited as a missed appointment fee for no-show or late cancellations. I understand that life happens, please communicate about last minute schedule changes so that we can work together on an alternative appointment time!
